How to access personal information

For an individual to access their personal information, a request in writing must be made to the Avenue 4 Privacy Officer stating specifically what personal information is requested.

Avenue 4 will respond to the request as soon as possible, and will advise if for some reason, we cannot respond immediately. We may charge you a reasonable fee for providing access to, or copies of, the personal information, and only after first advising of an approximate cost.

Can Avenue 4 deny access to personal information?

Avenue 4 has the right to refuse a request for access to personal information under any of the following circumstances:

Keeping personal information accurate

If any information needs to be updated or amended to reflect a change in an individual’s personal information, please let us know and we will update our records. Avenue 4 will amend personal information that is inaccurate or incomplete and, when appropriate, will endeavor to advise other relevant parties directly affected by such updates.

If there is a complaint, Avenue 4 wants to know about it
